Exceptional Circumstances Withdrawal

If you have exceptional circumstances that require you to request a withdrawal after the last day to withdraw for the quarter, please complete the Exceptional Circumstances Request Form.  Information regarding the process and supporting documentation needed is included on the form.  You may request a tuition refund using this form.  The form will be reviewed for approval; however, submission of the request does not guarantee a withdrawal or a refund.
