Environmental Health & Safety

The following is a list of EH&S forms that are provided to the University community to be used for notification and identification of various environmental and safety issues.  Click on the link and print out the form.  Fill in the form (print legibly) completely and submit it to EH&S either by fax 509.359.4690 or  101 HUS. 

In order to receive assistance from Environmental Health & Safety, all requests must be in writing on our "Service Request Form".  Your Supervisor or Residential Life Representative (if you are EWU Housing Resident) must sign and submit this form.  If forms are not submitted and signed, the work with not be conducted.
