This form is for new students who have an appointment with our office. Please print, fill out and bring with you to your appointment.

DSS Release of Information to third party

If you wish DSS to release your documentation to another institution, please fill this out and fax to our office. Needs student signature, addess and contact of third party, can be faxed or mailed.

DSS Exam Proctor Form

This is the test taking form that students/professors need to fill out and return with the test to the DSS office. We also have this form (which is yellow) in our office for students to pick up and take their professors.

Consent to Release Info

This form is for DSS to request your records from your provider. Needs to have student signature, can be faxed, mailed or hand delivered.

Alternative Text Request

This form is for students who have the alternative text accommodation. Please fill this form out as soon as you register to allow for processing time!

Taped Lecture Agreement

This form is for students who use tape/or digital recorders as an accommodation.
